Payment in full is due at the time of service, and will be collected at the end of session. Forms of payment accepted include cash, check, credit/debit.
Sliding Scale means that the fee you will pay for counseling services may be eligible to be reduced according to your total household income. Sliding Scale is often used when finances are an obstacle for treatment. If you wish to verify your eligibility for sliding scale, please be prepared to provide your most recent tax return and pay stubs.
Please Note: Total household income includes the amount of money your entire household brings in per year (or month) from potentially multiple sources including work income, child support, alimony, disability or any other regular income. If you are classified as a dependent, total household income includes parent/guardian income.
As an effort to safeguard your confidentiality, I am not an in-network provider for insurance. It is important that you understand that when insurance providers are utilized to cover counseling services, your confidentiality is limited because the provider will document your diagnosis as part of your medical record and can audit your records at any time to assess treatment goals and progress.
I accept all insurance plans as an out-of-network provider. This means you are responsible for the full session fee up front, but your insurance provider may reimburse you for a portion of that fee. Upon request, I can provide you with a copy of your invoice which will include your diagnosis for you to submit to your insurance company.
