In an effort to safe-guard your information, I am not contracted with insurance companies and therefore am considered an out-of-network provider. You will be provided an Insurance Reimbursement Form each month should you choose to submit a claim for potential reimbursement. Please understand your insurance policy will determine your eligibility for reimbursement.
